Butterbur (Petasites hybridus) extract is clinically studied for migraine prevention and allergic rhinitis relief. Only PA-free (pyrrolizidine alkaloid-free) extracts are considered safe, as raw butterbur contains hepatotoxic pyrrolizidine alkaloids that can cause severe liver damage.
Butterbur (Petasites hybridus) is a perennial plant whose rhizome extract contains petasin and isopetasin, sesquiterpene compounds that inhibit leukotriene synthesis, reduce vascular inflammation, and modulate calcium channel activity. These mechanisms make it particularly effective for migraine prevention, where clinical trials have demonstrated up to 48% reduction in migraine frequency at 75 mg twice daily. Butterbur has also shown efficacy comparable to cetirizine in relieving allergic rhinitis symptoms. CRITICAL SAFETY NOTE: Raw butterbur contains pyrrolizidine alkaloids (PAs), which are potent hepatotoxins that cause veno-occlusive liver disease and are potentially carcinogenic. Only commercially prepared PA-free extracts (such as Petadolex, certified to contain less than 0.08 ppm PAs) should be used. Unprocessed butterbur root, teas, or non-certified preparations must never be consumed. Some regulatory agencies have withdrawn butterbur products due to reports of liver injury even with purportedly PA-free extracts, so ongoing monitoring of liver function is recommended during use.
Two large randomized controlled trials demonstrated that PA-free butterbur extract (75 mg twice daily) reduced migraine frequency by up to 48% compared to placebo over 4 months. The American Academy of Neurology previously gave it a Level A recommendation for migraine prevention.
Clinical trials show PA-free butterbur extract is as effective as cetirizine in reducing nasal symptoms of seasonal allergic rhinitis without the sedative side effects of antihistamines.
