Policosanol is a mixture of long-chain aliphatic alcohols derived primarily from sugar cane wax. Cuban studies have reported significant cholesterol-lowering effects, though independent international trials have largely failed to replicate these findings.
Policosanol is a natural extract consisting of a mixture of long-chain primary alcohols, with octacosanol (C28) as the predominant component. Originally developed and extensively studied in Cuba, policosanol gained attention for reported effects on cholesterol metabolism comparable to statin drugs. The proposed mechanism involves inhibition of hepatic cholesterol synthesis at steps preceding HMG-CoA reductase, as well as enhanced LDL receptor expression. However, a significant body of independent research conducted in Germany, Canada, South Africa, and other countries has been unable to confirm the dramatic lipid-lowering effects reported in Cuban trials. This discrepancy has raised questions about methodological differences, source material variations, and potential publication bias. While policosanol appears to be well-tolerated with a favorable safety profile, its efficacy for cholesterol management remains controversial in evidence-based medicine.
Cuban clinical trials reported LDL cholesterol reductions of 20–30% with policosanol supplementation. However, multiple independent trials in other countries found no significant effect on LDL levels, making this benefit highly uncertain.
Several studies suggest policosanol may inhibit platelet aggregation by reducing thromboxane A2 synthesis and increasing prostacyclin levels, potentially supporting cardiovascular health.
Octacosanol, the main component of policosanol, has shown potential to improve exercise performance and reaction time in preliminary studies, though evidence remains limited.

Most studies used sugar cane-derived policosanol. Effects, if any, may take 6–8 weeks to manifest. Evening dosing aligns with peak hepatic cholesterol synthesis.
