Red yeast rice is a fermented rice product containing monacolin K, a compound chemically identical to the prescription statin lovastatin. Red yeast rice (RYR) is produced by fermenting white rice with the mold Monascus purpureus, yielding a product that has been used in traditional Chinese medicine and cuisine for centuries. The primary bioactive component is monacolin K, which is chemically identical to lovastatin, a prescription HMG-CoA reductase inhibitor (statin). Meta-analyses of randomized controlled trials have demonstrated that red yeast rice supplements containing standardized monacolin K content significantly reduce total cholesterol, LDL cholesterol, and triglycerides. A landmark Chinese trial (CCSPS) involving nearly 5,000 patients showed that a standardized red yeast rice extract (Xuezhikang) reduced recurrent cardiovascular events by 45% over 4.5 years. However, because monacolin K is pharmacologically identical to lovastatin, red yeast rice carries the same risk of adverse effects, including myopathy, rhabdomyolysis, and hepatotoxicity.
