Saccharomyces boulardii is a probiotic yeast with strong evidence for preventing and treating multiple forms of diarrhea. Unlike bacterial probiotics, it is naturally resistant to antibiotics, making it ideal for concurrent use during antibiotic therapy.
Saccharomyces boulardii is a non-pathogenic tropical yeast first isolated from lychee and mangosteen fruit in 1923. It is unique among probiotics in that it is a yeast rather than a bacterium, conferring natural resistance to all antibacterial antibiotics. Extensive clinical evidence supports its use in preventing antibiotic-associated diarrhea, traveler's diarrhea, and recurrent Clostridioides difficile infection. S. boulardii exerts its effects through multiple mechanisms, including secretion of proteases that degrade bacterial toxins, enhancement of intestinal brush border enzymes, stimulation of secretory IgA, and anti-inflammatory signaling. It does not permanently colonize the gut and is cleared within days of discontinuation.
Multiple meta-analyses demonstrate S. boulardii reduces the risk of antibiotic-associated diarrhea by approximately 50% when taken concurrently with antibiotics.
